I'd suggest OP that if you are wanting oral throughout the massage then you may be better thinking of your booking not as a massage with extras but an oral esocrt appointment with massage.

With the increasing price of extras in massage shops the differential isn't going to be as much as it was.

I'd agree. Massage extras are very expensive in some places. I've asked about 1hr massages for around £50 only to then be told HE is £30-40 and oral is another £30 on top of that. Want the girl naked? Guess what? Another £30-40. Unless you haggle (and that can kill the mood) the massage is costing the same as an escort booking. The difference is that the escort won't wait till the last 15/20 minutes on a 1hr booking.
I'd also say that many of the girls in the massage places have no idea what they're doing.  They're there to make money tossing clients off. If they don't upsell services, the boss won't happy. It can be common in some places to have a massage from a girl who has no idea what she's doing but will wank you if the money's right.
If you want oral throughout, you need an escort.
