When I've had the inclination to do something about it, I've found that the best thing to do is literally take it one day at a time and actually keep a record of how many days it's been since my last wank.
That way each day represents progress and it has made me more determined to keep going because I don't want to reset the clock and go back to zero.
Obviously, this approach has never worked indefinitely but it's certainly helped.
