Only offer 1 hour bookings. I know of somebody who stopped doing 15 minute booking precisely because of what you mention.

Some don’t mind the odd 5 minutes here and there,I don’t take the piss but I have seen a regular who never says anything if we go over,happened before in a 30 min booking turned into nearly an hour never complained or rushed.

shower time has been discussed before on here, some providers include it in the paid for time, some don't.
