Yes, I could perhaps understand the old, ugly and fat getting very desperate but even then it still beggars belief. Most women worry about things too much and get a bit paranoid - so why don't they get paranoid that the guys going to see them have STIs?! "Gift givers" (guys who try to pass on their HIV) do exist (although they will be rare - surely escorts are prime targets).
I can only presume that girls who offer it provide particularly shit service and feel the need to do "something" to make up for it (to get new/repeat clients). I don't think I'll ever understand.
