Girls at Lichfield do a very good job as you are kneeling on all fours.  (but maybe a bit too far for you??)

One of them, and I can't for the life of me remember her name,  gets you right on the bottom end of the table and kneels on the floor behind you..

Does a good job pulling the old boy as far back and out as she can. Closest thing to "milking" I've found.
