What happens in the room normally stays in the room. You pay the going rate (£40 or whatever to the desk) and the extras are dealt with in the room. The masseuse will probably ask you if you've been to the premises before (if you have say I have and say what extras you had and if you haven't the standard reply is I've been for loads of thai massages). This will help the masseuse assess if you're a legitimate customer or whether you'll be seeking extras. The "was I okay" question is the one where you should have broached the subject of extra services.
