From my experience at HoD places... 

If CIM & Rimming are on a girl's list, they aren't chargeable extras, but are offered at discretion, subject to the girl being happy with hygiene, etc.

Anal is chargeable at £40 maximum charge, again at discretion, due to sizing issues, etc.

Elise is pretty open-minded though.

Photos/filming are also extras (if the girl offers them). Price & content subject to negotiation with each individual girl, but I've been quoted £20 for stills showing face and hardcore action and £40 for filming the entire session from one girl.
