I'd not something I've noticed, but it sounds like something confined to the upper price tiers and/or the more specialised parts of the sex for sale market. Most SPs, Indy or otherwise,  have a set list of services they're willing to offer with a few of them like Anal charged as an "extra".

Extras and such I get. And makes sense. But not having any idea of rates at all.

Could be like different folks get different rates. Wild swings on them. Seems a shame

I would suggest bespoke is simply the posh way of saying extras cost more. 🤷🏼
