partially a tattoo, but there's an operation scar there as well. Also either a strawberry-type birthmark or an extreme version of the pigmentation marks some women get when pregnant.

I think it's a tattoo being used to cover up a surgical scar.  I've seen a similar scar before.

True. Some of these Thai girls who have had surgery in Thailand are left with pretty gruesome scars
Years ago there was a Thai girl in Telford known as Suzi Thai who had a big scar. No tattoo but she was very conscious of it.
