However IMHO recently there seems to be, certainly in the London Boards (general section pages, but esp in the TS sections) a lot more new members / guys very open and even blasé about BB'ing.

Now each to their own, their life and lifestyle. They can, and obviously do, choose to partake in sex anyway they like, but there seems to be an uptick.

The other thing that amazes me (or even shocks me) is the lack of understanding about sexual health that is apparent in some of the posts. There 100% seems to be a misconception that PREP protects for anything other HIV. Yes a girl/TS maybe on PREP but that will not protect you from any other STI.

They are not scare stories. Antibiotic resistance is an issue. There is very little in the way of development of new antibiotics, mainly because it just not economically viable for the pharma industry.

Currently the standard treatment for gonorrhea is intramuscular ceftriaxone, however, there are strains that are antibiotic resistant and this presents a public health crisis as there aren't many effective treatments remaining, which is concerning when prevalence is increasing.

HPV is serious, as it has long term implications, given its relationship with cervical, throat, and mouth cancers. There is a reason why there is an active vaccine programme, which has extended to teenage boys as well. With bisexual men under the age of 45 being offered the jab as well.

Sad to say people are less scared these days. Especially the younger generations who may have received HPV vaccines and think they are gods.

I am the generation who missed the vaccine by many many years because at that time it wasn't even on the market, and I am well aware of how much damage high risk HPV can cause to women ( cervical cancer or laser treatments etc. ). So using condoms is necessary.

Problem is Gardasil doesn't protect from every strain anyway. And the later you take it the less of an effect it has as you would have already been exposed to HPV from just having sex with a long term monogamous partner. So that's for HPV.

Two months ago I went to DSE to get a Hep B booster and when they did assessment of my activities they offered me to be on PREP. I said I didn't want it because I was only doing owo and dont bareback, good quality condoms and lube are a must. She said to me they routinely offer it to escorts. I wasn't happy to hear this because I felt it was encouraging risky behaviour. Only just ten years ago something like this wasn't available. I don't agre me with Prep being dished out unless someone had a condom accident or has been subject to violence, but times have changed I guess.

Problem is it's easy to blacklist someone when you find out they have seen a barebacker or been barebacking.......but what if you don't know? In the trade you cannot know what people are doing when they aren't seeing you. When there is evidence then you can at least make an informed decision, but what if there isn't any? I feel saddened by some of the posts but what can you do?

It's possible in a few years time throat cancer cases related to high risk HPV will rise to the point where only a small amount of escorts will be willing to offer owo. We shall see but yeah.

Two months ago I went to DSE to get a Hep B booster and when they did assessment of my activities they offered me to be on PREP. I said I didn't want it because I was only doing owo and dont bareback, good quality condoms and lube are a must. She said to me they routinely offer it to escorts. I wasn't happy to hear this because I felt it was encouraging risky behaviour.

I am very surprised at that. There is scope in the guidelines to assess each case individually, but being a sex worker alone does not really fit unless there is also condomless penetrative sex. (see below) I would tend to agree with you that such prescribing encourages risky behaviour. I would be interested to know if other SP's have been offered this at clinics?

In addition and I have posted this many times before Hep B Boosters are also not required, yet it seems GUM clinics still do so. It is an example of the NHS not sticking to guidelines and providing unnecessary treatment paid for by the taxpayer.

Yes it shocked me kind of. She said it very casually as well. Maybe they assume you don't care about yourself, you drink and don't know what is going on, you have ton of condom breaks etc. Well this is not my case and they are dishing it out like candies and they don't even know what the long term effects of being on Prep are like towards your liver or kidneys. It's not been around for that long.

And this was the first time I was ever offered it at DSE as well. I guess they are very HIV wired. Could be just the stigma/myths towards escorts.

I haven't been offered PREP I have heard of sex workers being prescribed it, but I do not have any factual accounts only hearsay. Regards Hep B the clinic I use does periodically decide my immunity is too low and give me a booster, hep b immunity is included in the suite of tests offered to me during my routine screening.

It varies from clinic to clinic. I usually do SHL and I am pretty sure if you tick that you drink, take drugs, bareback etc. they would offer you prep as well.

Dean Street are just "obsessed" with HIV because it was originally opened to tackle the rising cases in London amongst the Soho gay community. Maybe they assume a lot of punters bareback too, or have sex with other men and escorts will be exposed to this risk as a result. I only go because of quick results as I feel they don't cater for escorts as much as they once used to.

But they are still ace in terms of speedy results and they are now offering appointments to anyone, with much shorter waiting time. So if anyone wants to get tested via Dean Street, they can offer them an appointment regardless of being in the sex industry or not.

Yes when I went to Dean Street for a blood test ( I can't stand needles anywhere near me I have to say ) so that's always something that makes me feel unwell. Then they sent me a text few hours later all okay but Hep B immunity low and that I need to make a follow up. I didn't have time to do it immediately so I left it a month or so and then the nurse tells me the body has got a memory of some sort and doesn't need it. But she told me that after the injection. Had I known before I would have said no thank you. Plus the costs if it's really not needed. Same as Prep. How much does that cost to give it to people who don't really need it. And the long term affects what.

I know Ruth and we have had this conversation before and I always post the same link  ;)

There is no indication for repeated antibody testing beyond the immunization schedule apart from healthcare and laboratory workers as part of occupational health/HSE requirements. There is no need for a booster other than a five year one for that same group. This has been this way for quite a number of years, yet Sexual Health Services seem to still go against the guidelines?

There may also be some immuno-compromised groups who may require a different protocol.

My thoughts too based on zero medical training my logic says if it's that effective at preventing HIV getting a toe in, when else is it doing? I know that is based on knowing nothing but a barrier e.g. condom seems safer on balance.
