Sildenafil and Tadalafil are only licenced for Pulmonary Arterial Hypertension which is a relatively rare cause of hypertension. Is that your diagnosis?

Morning Doc  :hi:.

Are you saying Sildenafil isn't licensed for treatment of erectile dysfunction in the UK?

I said that in my own experience, I found Viagra to work better... i.e. for me.  If you take the difference in composition for Viagra and Mylan branded Sildenafil for example, the latter does not use lactose as a part of it's inactive ingredients i.e. a bulking agent, wheres as Viagra does use it.  Lactose can affect individuals in different ways (not necessarily lactose intolerant) and can affect how medication is absorbed.
I've just checked my packs of Sildenafil and the ingredients are exactly the same as 'genuine' Viagra, including lactose. I'm not sure if you are aware but lactose is only used in the film coating, not as a bulking agent.

The generic version supplied to me are manufactured by ALP Swift Services (Malta) and distributed in the UK by Milpharm Ltd, who are the Marketing Authorisation Holder.

The problem with sildenafil (be it Viagra or generic) is its hugely variable absorption rate. This the main reason people report variations in response. Tadalafil is much better in this respect because its absorption is much less effected by stomach contents.

Interesting - I have noticed sildenafil hasn't been very effective after a rather heavier than usual lunch. I've used it as an "insurance policy" on punts, with a low dose (25mg) but as tadalafil is available very cheaply online now I might try it more. It is certainly longer-lasting than sildenafil, helps if you are not sure when you might be in action!

Regarding non active ingredients etc in Sildenafil these are of no real significance in terms of effectiveness of the active ingredient. The popular form of course is very marketable, easy to package and keeps well. If they started selling the chewable mint tablets that have gone down very well in some countries and which are absorbed a bit faster, freshen the breath, and are not quite as stable once out the packet, they'd be accused of "pandering to party-people".

You are quite right about dosage by weight but it is a complex subject. However it is not really of any practical relevance for Sildenafil unless in paediatric prescribing.

The recommended adult starting dose is 50mg then either increasing to 100mg or reducing to 25 mg depending on outcome and tolerance etc.

The exception to this is a recommended starting dose of 25mg for over 65s or with the presence of certain underlying conditions or concurrent medications.

Yes I tried the imported (unlicenced) chewable versions years ago (in particular tadalafil) as well as the jellies. As you say does not seem to be a UK market for them?
