I'm  not sure I'd agree with the OP's assumption that long term use of Viagra leads to reduced effectiveness. He may be drawing a false analogy with antibiotics.
Be that as it may, the risks in 'hotdogging' (every day is a school day) involve specific STIs, namely syphillis (thankfully still rare in heterosexuals), genital warts and molluscum contagiosum.
Perhaps someone with real medical knowledge would care to comment.

This is another one where we are into theoretical largely unproven risks, but overall this is considered a low risk activity.

Strictly speaking hotdogging is penis between bum cheeks. Genital to genital contact is often also referred to as frotagge, although this in turn more usually refers to penis to penis contact which many gay men practice because it is considered a very low risk activity.

Penis to vulva contact is considered low risk. HIV is considered almost zero. Infected vaginal fluids could pose a risk of other STIs eg clamydia but once again most authorities consider this low risk of transmission.
The greater risk is with Herpes and HPV viruses and to a degree syphilis which do not rely on bodily fluid transmission but on skin to skin contact. Of course even with a condom on there is still a risk in the case of these viruses.

If the activity leads to penetration for any length of time then the goal posts are moved slightly.

I would ask your doctor, what's ok for him might not be for you. I'm on heart meds and my doc did 7 different blood tests before he prescribed it for me.

I would endorse seeing a doctor 100%. I am on BP meds and am lucky ED pills don't affect me too much but for me Calais is better as there is no flushing, headache etc and more effective. but that's me YMMV
If you are of a "certain age" they are free on script anyway.
