Personally I've found the indies and most agency girls are usually very well presented. However, when girls are doing back to back bookings I've found it's not so great.

Obviously you can. When you noticed she wasn't fresh you should have asked her to shower. Then again, it's easier said than done.
On the few occasions a guy has stunk, I was unable to bring it up and I just counted the minutes until time was up. Luckily it hasn't happened in a while. The next time it happens, I'm just going to have to say something.

For some people if their only shower in a week is the two minute quick wash before their punt, they are going to reek regardless of the wash. Some smells are ingrained.

I don't actually encounter many smelly clients, mine are generally a respectful and clean bunch, but I wish some guys  would remember to wash their bums properly during a shower and underneath their balls. Some of us might not do anal play but we can still smell it during oral.

I just generally there are some people with bad hygiene.
