Either try pulling out before going soft or making sure the condom fits better when hard.  A condom should not slip off if your penis is still erect unless it is a badly fitting one.  It may unroll if too small or slide off if too big.  Perhaps she had run out of normal condoms and only had one size left?

When I was 18, yes. Unless you are wearing the wrong size condom (in which case you should take a selection of your own for the prossie to choose from), it happens when cock is enjoying relaxing into post-ejaculation floppiness. Either, take it out first (earlier!) or do what a number of WGs will do anyway: grip the condom at its base (the bit you can still reach while its inside her) with the thumb and forefinger and keep a hold of it as you withdraw.

A lot of older cheap condoms (especially some of the short arse Durex brands) are not particularly long. If you have a long dong, you need something more than a bit of mini-cling-film-condom over the end of it: you need something you can roll down over most or all of the shaft.

There's a number of good options. One of the discerning choices is Skyns (sure, they cost a bit more, but a fraction of the cost of the punt).
